Operator must obtain a public food service license from the Division of Hotels and Restaurants within 60 days. Submit a license application and payment to DBPR/Bureau of Central Intake Unit, 2601 Blair Stone Rd., Tallahassee, FL 32399-0783. To apply online or obtain an application for license visit https://www2.myfloridalicense.com/hotels-restaurants/. Unable to license establishment due to placed on multiple warnings.
  • High Priority Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Hose Bibb by hand wash sink on left side of cookline missing vacuum breaker.
  • Intermediate No paper towels or mechanical hand drying device provided at handwash sink. Paper towels missing from hand wash sinks.

  • Basic Outer openings not protected with self-closing doors. Self closer disconnected at side door to kitchen.
  • Basic Standing water in floor drain/floor drain draining very slowly. Mop sink outside back kitchen is slow draining with standing water observed in curbed area. Also, no hot water to sink.
  • Basic Water leaking from pipe and/or faucet/handle. When full with water, far right bay of triple sink leaks onto floor.
