JACKSONVILLE, Fla. - Hot Wok on Emerson Street failed its third consecutive restaurant inspection on Oct. 23 after inspectors found five live roaches and additional evidence of roach activity. China Wok on Deerwood Lake Parkway and the El Fogon De Lily food truck passed their emergency order callbacks that same day, with inspectors recording zero violations at both establishments. The results were among 44 restaurant inspections conducted across Northeast Florida, including seven administrative complaint recommendations and eight warnings.
The latest inspections follow emergency order recommendations and callbacks covered in the Jax Times Sunday restaurant roundup. Florida inspection records distinguish between an emergency order recommendation, a callback that does not comply and a callback that finds the problems corrected. The latest results show different outcomes for businesses that faced enforcement actions during the previous inspections.
Hot Wok Fails Third Consecutive Inspection
Hot Wok, located at 3715 Emerson St. in Jacksonville, received an "Emergency Order Callback Not Complied" result on Friday, Oct. 23. That marked the third consecutive day the restaurant failed to meet inspection requirements, following an emergency order recommendation on Oct. 21 and an unsuccessful callback on Oct. 22. Inspectors documented live and dead roaches, along with egg sacs, during the latest visit.
The Oct. 23 inspection identified five live roaches in different parts of the restaurant. Two were on the floor beneath the handwashing sink in the back kitchen, one was beneath bulk dry goods storage on wheels, one was on metal shelving posts near dry goods storage and another was on the floor beside a gray stand-up reach-in cooler. The person in charge killed and discarded all five live roaches during the inspection.
Inspectors also found dead roaches and egg sacs in storage and preparation areas. One dead roach was on shelving at the front counter, another was on top of a bulk rice container, and a third dead roach and an egg sac were on shelving beneath a preparation table. Inspectors identified another egg sac on shelving in dry storage but reported no flying insects.

Hot Wok has faced a similar inspection sequence before. On Aug. 17, 2026, inspectors recommended an emergency order, followed by an unsuccessful callback on Aug. 18. A callback on Aug. 19 found the restaurant complied, but the latest October records show another series of unresolved inspection findings.
China Wok and El Fogon De Lily Pass Callbacks
China Wok, located at 4320 Deerwood Lake Pkwy., received two callback inspections on Oct. 23. The first resulted in an "Emergency Order Callback Not Complied" finding because a high-priority roach violation remained from the initial inspection. A second callback conducted the same day resulted in "Emergency Order Callback Complied," with inspectors recording zero violations.
The El Fogon De Lily food truck also passed its emergency order callback on Oct. 23. The business, licensed at 2356 W. Beaver St., received an "Emergency Order Callback Complied" result with zero violations. Both establishments were among the businesses identified in the earlier roundup of Oct. 22 inspection activity.
Three additional establishments that received administrative complaint recommendations on Oct. 22 passed callbacks on Oct. 23. Garden Street Bistro and Back Porch Southern Eats in Fernandina Beach each recorded zero violations, as did Broskii Fish & Chicken in Jacksonville. Those callback results indicate inspectors found the previously identified problems corrected during the follow-up visits.

The inspection records provide both enforcement outcomes and individual violation descriptions. Some businesses received follow-up inspections after previous enforcement recommendations, while others were cited for food temperatures, sanitation practices or equipment-related issues. Florida does not issue restaurant letter grades, so the inspection results and documented violations provide the information available to consumers.
My Arepa Has 6 High-Priority Violations
My Arepa11, located at 1712 Ogilvie Park Ln. in Jacksonville, received an administrative complaint recommendation after inspectors recorded six high-priority, two intermediate and three basic violations. The inspection report describes a truck and documented problems involving water, wastewater disposal, food temperatures and food storage. The person in charge corrected some water-related conditions during the inspection, but inspectors also documented food safety concerns.
Water ran out during the inspection, prompting the person in charge to refill the tank. Inspectors also observed wastewater draining onto the ground through a hose while water was in use, and the person in charge closed the hose so wastewater drained into the tank. Other findings included raw shell eggs and quail eggs stored at a room temperature of 72 degrees Fahrenheit since Monday and Tuesday, resulting in a stop-sale order.
Inspectors also found cheese and cut tomatoes at temperatures ranging from 44 to 54 degrees Fahrenheit. Raw eggs and raw sausage were stored above ready-to-eat items, creating another high-priority concern. The inspection additionally found no proof of employee food safety training.
Six Other Businesses Face Administrative Complaint Recommendations
Chans Restaurant17 at 842-3 Dunn Ave. received an administrative complaint recommendation with two high-priority, two intermediate and 13 basic violations. Inspectors reported an employee moving cooked noodles with bare hands, raw chicken stored above ready-to-eat pork and a handwashing sink used for dumping water. The chicken storage and handwashing sink findings were repeat violations.
Jersey Mike's Subs at 3541-1 University Blvd. W. received two high-priority violations, both repeat findings. Inspectors recorded deli meat and cheese at 44 to 45 degrees Fahrenheit in a display case and a missing vacuum breaker at the mop sink. Panera Bread #861 at 13201 Atlantic Blvd. also received two high-priority violations, including a cashier handling clean knives after working at the takeout window without washing hands and a repeat backflow preventer issue at the mop sink.

Grazing Parlor0 in Jacksonville Beach and Vilo's Cafe0 also recorded zero violations, with Lundy Caribbean Restaurant and Vilo's Cafe undergoing licensing inspections.

What Florida Restaurant Inspection Results Mean
Florida classifies high-priority violations as conditions that could directly contribute to foodborne illness or injury. Intermediate violations involve conditions that could lead to high-priority risks, while basic violations concern best practices. These categories help distinguish the seriousness of inspection findings.
An "emergency order recommended" result means an inspector recommended immediate closure until problems are corrected. An "Emergency Order Callback Complied" result means a follow-up inspection found the problems corrected, allowing a business to reopen. These records do not establish whether any specific establishment is open today, and readers should check current information rather than assume its operating status.
Consumers can search restaurant inspection histories and follow establishments for alerts at https://jaxtimes.com/restaurants.
